qualified Home Health RN in New York City, New York, 10038! Job
Title: Registered Nurse (RN) Profession: Nursing Specialty: Home

within the last ten (10) years License: Current RN Licensure in New
York State Certifications: BLS required before the start date
Must-Have: - Two (2) years of experience in a home setting within
the last ten (10) years - Recent experience conducting
comprehensive assessments of patients Description: The role
requires conducting initial in-home pre-evaluations of patients.
Nurses will evaluate health status, strengths, care needs, and
preferences. This evaluation guides the development of
individualized long-term care service plans. Candidates will review
consumer medical documentation and relevant health referral forms.
Evaluation data must be entered into electronic evaluation forms
and transmitted as required. Nurses will document any concerns or
conflicting information that surface during the evaluation process.
Emphasizing continuity of care is essential to reduce or eliminate
fragmentation, duplication, and gaps in treatment plans. Candidates
will discuss healthcare options, necessary supports, service vendor
options, and waiver options with consumers. Initiating
communication between the Call Center and eligible consumers for
enrollment into managed care health plans is required. Enhancing
communication and collaborative relationships with
interdisciplinary care team members improves care coordination and
facilitates service delivery. Collecting quality review data and
any required documentation to support outcome measurements and
recording case notes into CRM is necessary. Nurses will identify
opportunities for health promotion and illness prevention.
Maintaining a comprehensive working knowledge of community
resources and payor requirements is essential. The role requires
meeting all standards established for this position as outlined in
annual performance criteria. Other duties may be assigned by the
