The PT will evaluate and treat patients to improve mobility, strength, and overall functional independence. This role involves collaborating with interdisciplinary teams to deliver high-quality rehabilitation care and achieve optimal patient outcomes. Key Responsibilities Evaluate patients physical conditions and functional limitations Develop and implement individualized treatment plans Provide therapeutic exercises and rehabilitation interventions Monitor patient progress and adjust care plans as needed Educate patients and caregivers on recovery and injury prevention Maintain accurate and timely clinical documentation Collaborate with healthcare teams to coordinate patient care Ensure compliance with facility policies and regulatory standards Requirements Active Physical Therapist (PT) license in the state of New Jersey Degree from an accredited Physical Therapy program (DPT or equivalent)
