Meal Preparation: Plan and prepare nutritious meals tailored to dietary needs. Household Assistance: Maintain a clean, safe, and healthy environment for patients. Documentation: Accurately record patient care activities using Celltrak. Team Collaboration: Report changes in patient conditions to RN Case Managers. Qualifications Certification: Valid NJ Certified Home Health Aide (CHHA) in good standing. Communication Skills: Proficient in reading, writing, and communicating fluently in English.
